Where can your adjustments be recorded?

You can ask healthcare professionals to record your reasonable adjustments in one or both ways:

  • Alert on your patient record: A note can be added to your medical record so your adjustments are visible to staff.

  • Reasonable Adjustment Digital Flag: You can also ask for your adjustments to be recorded on your Reasonable Adjustment Digital Flag, so they can be seen across different NHS services.
